ACE (angiotensin-converting enzyme) is a substance that plays a leading role in the regulation of blood pressure on the walls of blood vessels ACE (angiotensin-converting enzyme) is a substance that plays a leading role in the regulation of blood pressure on the walls of blood vessels.

His discovery led to the creation of a whole group of pharmaceutical preparations, now widely used in the cardiology industry - ACE inhibitors, one of which is Prestarium A.

Studies of the drug have shown that it reduces the risk of developing strokes, complications in acute myocardial infarction, as well as heart attacks themselves.

"Prestarium A" begins to act 5 hours after admission , while active suppression of the ACE continues for a day or more. Discontinuation of the drug is not accompanied by withdrawal syndrome.

Perindopril, which is part of Prestarium A, suppresses the operation of the ACE, thereby achieving the following clinical effects:

  • Decreased blood pressure (both systolic and diastolic);
  • Increased cardiac output and accelerated peripheral blood flow (without an increase in heart rate);
  • Increased blood flow in the kidneys;
  • Restoration and increase of elasticity of vessel walls;
  • Reduction of left ventricular hypertrophy.

Instructions for use

Composition and release form

"Prestarium A" is produced in the form of tablets in a film coating, having a rounded biconvex shape, depending on the mass, white, light green or green.

There are three possible dosages of the drug: 2.5 mg, 5 mg and 10 mg with the appropriate content of the active substance - perindopril arginine.

In addition to it, the composition of the drug includes additional chemical compounds, providing the appearance of tablets and some pharmacokinetic indicators:

  • Lactose monohydrate;
  • Magnesium stearate;
  • Colloidal silicon dioxide;
  • Maltodextrin;
  • Sodium carboxymethyl starch;
  • Additives E422a, E464, E171, E141;
  • Macrogol 6000;
  • Copper chlorophyllin (tablets 5 and 10 mg).

One pack contains 30 tablets placed in a polypropylene bottle.

Indications

"Prestarium A" is indicated for patients suffering from cardiovascular diseases, namely:

  • Arterial hypertension syndrome;
  • Ischemic heart disease;
  • Disorders of cerebral circulation (including patients who have had a stroke);
  • Chronic heart failure.

Arterial hypertension syndrome; Ischemic heart disease; Disorders of cerebral circulation (including patients who have had a stroke); Chronic heart failure

Mode of application

"Prestarium A" is taken once a day, in the morning, before meals. The dosage required by the patient is selected by the doctor under close medical supervision: the therapy is started with minimal doses, increasing them under the condition of normal tolerance.

The maximum allowable daily dosage is 10 mg. It is allowed to use "Prestarium A" as part of a combination therapy.

Drug interactions

When using "Prestarium A" together with aliskiren significantly increases the risk of increased levels of potassium in the blood and, as a consequence, impaired renal function and the likelihood of death of cardiovascular etiology.

Also, the risk of hyperkalemia increases when taking perindopril in conjunction with potassium preparations, potassium-sparing diuretic, nonsteroidal anti-inflammatory, angiotensin receptor antagonists, immunosuppressants (especially with trimethoprim, cyclosporine), heparin, and other ACE inhibitors.

In patients suffering from chronic heart failure, vascular atherosclerotic changes, diabetes mellitus, dual therapy with Prestarium A and drugs from a number of angiotensin II receptor blockers can lead to excessive pressure reduction (including drastic) and impaired renal function (up to acute renal failure).

A one-time intake of perindopril and estramustine, or of a drug from the group of gliptin (saxagliptin, linagliptin, vitagliptin, sitagliptin) is capable of causing Quincke's edema in a patient A one-time intake of perindopril and estramustine, or of a drug from the group of gliptin (saxagliptin, linagliptin, vitagliptin, sitagliptin) is capable of causing Quincke's edema in a patient.

Reception "Prestarium A" against the background of therapy with lithium preparations leads to an increase in the concentration of lithium in the blood plasma and the occurrence of intoxication related to this, which is accompanied by severe neurological disorders.

Insulin and other drugs designed to reduce blood glucose levels , together with Prestarium A, have a more pronounced effect - there is a risk of developing hypoglycemic coma.

Patients to whom Prestarium A was prescribed on the background of intravenous production of gold-containing drugs (in particular, sodium aurothiomalate) showed a kind of symptom complex, manifested in a strong drop in blood pressure, skin hyperemia, nausea and vomiting.

Baclofen, vasodilators (including nitroglycerin), neuroleptics, tricyclic antidepressants, anesthetics enhance the hypotensive effect of perindopril. Sympathomimetics and non-hormonal anti-inflammatory drugs, on the contrary, reduce the effectiveness of Prestarium A.

Precautionary measures

Contraindications

The drug is strictly prohibited in the following diseases and conditions:

  • Quincke's edema (hereditary or in history);
  • Hypersensitivity to perindopril or other ACE inhibitors;
  • Pregnancy (possible deviations in the development of the embryo);
  • Lactation;
  • Age up to 18 years;
  • Lactose intolerance.

In some cases, the use of "Prestarium A" should be treated with extreme caution.

These include:

  • Bilateral narrowing of the arteries of the kidneys;
  • Renal failure;
  • Systemic diseases of the connective tissue (scleroderma, lupus, etc.);
  • Taking procainamide, allopurinol, immunosuppressants;
  • Decrease in the volume of the circulating blood;
  • Cerebrovascular pathology;
  • Angina pectoris;
  • Diabetes;
  • Renovascular increased blood pressure;
  • Surgical intervention under general anesthesia;
  • Kidney transplant;
  • Hyperkalemia;
  • Hemodialysis;
  • Heart defects.

Side effects

In the course of research of the drug "Prestarium A", it was found that the following side effects most often occur in patients:

  • Headaches, dizziness;
  • Gastrointestinal disorders (diarrhea, constipation, vomiting, abdominal pain);
  • Noise in ears;
  • Blurred vision;
  • Paresthesia;
  • Excessive fall in blood pressure indicators;
  • Cough;
  • Skin itching, rash;
  • Asthenia;
  • Muscle spasms.

In addition, a number of phenomena occurring with a lower frequency were recorded:

  • Increased activity of bilirubin and liver enzymes;
  • General weakness;
  • Fever;
  • Edema;
  • Erectile dysfunction;
  • Lack of renal function;
  • Pain in the muscles and joints;
  • Hives;
  • Angioedema;
  • Dry mouth;
  • Bronchospasm;
  • Dyspnea;
  • Increase in heart rate;
  • Vasculitis;
  • Sleep disturbances;
  • Increased levels of potassium in the blood;
  • Decreased blood sugar
  • Eosinophilia.

Overdose

Suspicion overdose perindopril allow such signs as:

  • Severe pressure drop;
  • Shock;
  • Lung hyperventilation syndrome;
  • Violations of the heart rate (possibly as an increase in, and contraction);
  • Kidney function disorder;
  • Anxiety;
  • Dizziness;
  • Cough.

When they appear, the patient should be immediately hospitalized in a medical facility.

Treatment of overdose "Prestarium A" consists in removing the active substance and its metabolites from the body, which is achieved by washing the stomach and using adsorbing agents. If necessary, hemodialysis is performed, an artificial pacemaker of the heart is installed, pressure-increasing medications are introduced.

Special instructions for use

In patients at risk for the development of serious side effects, during the treatment of Prestarium A, it is necessary to conduct regular monitoring of blood pressure, renal function, and potassium levels in the blood.

In individuals receiving perindopril, along with hemodialysis through high-flow membranes, there have been cases of severe anaphylactic shock In individuals receiving perindopril, along with hemodialysis through high-flow membranes, there have been cases of severe anaphylactic shock.

With the development of angioedema, the “Prestarium A” treatment should be stopped. If you need to perform a surgical intervention, the drug is canceled the day before the operation.

Due to the fact that, against the background of the treatment “Prestarium A”, fainting and dizziness may occur, people who drive vehicles or perform highly precise types of work should be discarded.

If a patient develops arterial hypotension on the background of the drug intake, it should be laid on his back with the headboard down, in severe cases, it may be necessary to replenish the BCC with sodium chloride solution or the introduction of special medicines.

Analogs

Among the ACE inhibitors can be distinguished several drugs based on perindopril, similar to "Prestarium A":

All of them have a lower cost than the desired drug, but do not always demonstrate sufficiently high performance indicators. If perindopril is intolerable, you can pay attention to other drugs from this pharmaceutical group - for example, drugs based on captopril , enalapril , lisinopril .

Reviews

Feature antihypertensive drugs lies in the fact that they act on all patients Feature antihypertensive drugs lies in the fact that they act on all patients a little different a little different.

That is why the reviews about the "Prestarium A" are diametrically opposed - some people consider it the best pressure reducing agent , he did not help others at all and forced him to continue the search for "his" antihypertensives.

If we talk about side effects, then most often patients complain of a dry cough that occurs after a long period of taking the drug.
